CHART MANAGER

To assign notes from a visit to a patient's data, choose the Notes button in the Patient Manager window. Doing so brings up the following window.

This is a listing of electronic notes that have been entered into a patient's chart. To begin a new note, click on the button in the toolbar. This will bring up the Notes Data Entry window. (To read about entering new notes, click here.)

Notes are sorted by date in descending order. Scroll up or down through the list to retrieve the desired information.

The icon next to the note indicates the note's status; a check mark ([cm]) indicates a normal note.
A [fo] indicates that the note has been followed up.
The [co] icon, along with a red background, indicates that a note has been crossed out and is not valid.
The [let] icon shows that a letter has been issued regarding the matter in the note.
The [ref] icon shows a referral regarding the note.

BUTTON DESCRIPTIONS

This will re-build the list of notes to reflect the most current changes made to the list.
This will bring up the Phrase List window.
This button will bring up the selected patient's ledger. The ledger is the patient's financial transaction file. It stores and tracks all payment made by the patient.
This button accesses the patient's E-Chit. The patient's E-Chit is separate from the ledger; it stores specific information about the type of procedure performed.
This indicates a follow-up to the currently selected note. First, select the entry that requires a follow-up, then click on this button.

Followed-up items have the [fo] icon beside them.

Use this button to cross out (erase) a note. First, select the note to cross out, then click this button.

The "cross-out" method is an alternative to erasing an entry in the patient's record. Use it to indicate that a note is not valid.

Crossed-out notes appear with a red background and have the [co] icon beside them

Select a note and click on this button to indicate that a letter has been sent regarding the matter discussed in the current note.

Notes that have had letter sent about them have the [let] icon beside them.

This button indicates a referral. Select a note and click here to mark the current note as a referral.

Notes marked in this manner have the [ref] icon next to them.

ENTERING A NEW NOTE

Click to begin a new note. The patient's name and the current date are automatically filled in. There are two ways to enter the note text itself: it can be typed directly into the lower field or it can be selected from a pre-determined list (To read about the Phrase List, click here). If selecting from the phrase list, click the desired message in the Phrase List window and hit the Accept button. The selected phrase will appear in the Notes Data Entry window.

When the note is complete, click on the button in the toolbar. Each note is automatically signed according the to current user.

THE PHRASE LIST

Click on the [phrases] button in the toolbar of this window to access a list of phrases stored within the program that allow quick entry of frequently used notes. The Phrase List window lists such phrases.

If selecting a phrase for entry, hilite the desired phrase and click on the Accept button.

Otherwise, if you wish to enter a new phrase, click the button in the toolbar of this window. This will present the Phrase Entry window. Click again and type the new message into the field. Click the button in this window to add the note to the list.
